Medication Consent Form

PLEASE REMEMBER: NO MEDICATION OF ANY KIND IS TO BE TRANSPORTED TO SCHOOL

BY A STUDENT. PARENTS/GUARDIANS MUST BRING MEDICATION TO SCHOOL AND

ENSURE A SIGNED CONSENT IS ON FILE IN THE CLINIC.

• Please remember that your student must be without fever for a full 24-hours without the use of fever-reducing medicine (Tylenol, Advil, Motrin, etc.) before they can come back to school after being sick! 

• You can find helpful information in the link below from Children's Healthcare of Atlanta.  This link may help answer questions about whether your child is ready to come back to school or not. You're always welcome to call the school nurse's clinic if you have any questions about whether your child is ready to come back to school.

Primary Care of Southwest Georgia, Inc., in partnership with the Thomas County School System, is providing medical services for students, faculty and staff.  Located at Thomas County Middle School, the health center has a separate front entrance, and offers appointments and after-hours support.  Primary Care of Southwest Georgia, Inc.  provides quality healthcare regardless of ability to pay via a discount program.  The center accepts Medicare, Medicaid, most commercial insurance plans, Mastercard, and Visa.
